HOW TO MAKE A REFERRAL
Refer to CLASS
To refer to CLASS you first need to go to your local authority panel and gain approval for referral. Once you have approval:
- if you are referring for a specific therapy without initial consultation complete the Panel referral form and send this form to classctm@thebehaviourclinic.com
- if you are referring for an initial consultation book a consultation below or call us on 03301334977to book a consultation. Once booked complete send the panel referral form to classctm@thebehaviourclinic.com
CLASS Consultation Clinics are held every Thursday 9am to 5pm. Book in below.
